Agents can set visibility on first publish

Agents can now choose an app's visibility (public, unlisted, private) on the first publish. When publishing changes a live app's visibility, the message tells the agent exactly what changed (e.g. "The app is now unlisted (it was public)"). Agents cannot publish private apps.
